Liverpool Care Pathway (LCP)
The Liverpool Care Pathway for the Dying Patient (LCP) has been developed in the UK to transfer the hospice model of care into other care settings. It is a multi-professional document that provides an evidence-based framework for end of life care.
The LCP provides guidance on the different aspects of care required, including comfort measures, anticipatory prescribing of medicines and discontinuation of inappropriate interventions. Additional psychological and spiritual care and family support are included.
Judith Seagrave, Macmillan Nurse/LCP Project Lead, is in the process of rolling out the LCP in the Selby & York community setting. An LCP Champions group of District Nurse representatives has been set up to assist in the cascading of LCP education and audit.
